Crown Lengthening

Crown lengthening is a specialized periodontal procedure in which a portion of the gum tissue (gingiva) is removed for aesthetic or functional reasons, and if necessary, the bone around the tooth is reshaped or reduced.

 

The goal is to extend the visible part of the tooth to enable high-quality restorative or prosthetic treatment and to improve the overall aesthetic harmony of the smile.

Gummy Smile

When is crown lengthening needed?

 

This procedure is recommended in the following situations:

 

  • Deep caries or tooth fracture below the gum line

  • Aesthetic reasons – when the patient has a so-called “gummy smile” (excessively visible gums while smiling or speaking) or uneven gum levels on the teeth

  • Pre-prosthetic reasons – when there is not enough tooth structure for stable crown placement

  • Violation of biological width – when the distance between the margin of the existing crown and the bone is too small, which can lead to inflammation or gum recession

 

Besides improving the aesthetics of the smile, crown lengthening can prevent tooth loss and allow for more stable and long-lasting prosthetic reconstructions.

How does the crown lengthening procedure look?

 

Depending on the condition of the tooth and the therapeutic goals, the procedure may require a multidisciplinary approach. In cases of deep caries, the defect is treated immediately during the procedure, while the gingiva is corrected at the same time during tooth preparation for prosthetic work.

 

The procedure involves precise removal of gum tissue, either by laser or scalpel, followed by bone reshaping (recontouring) and/or bone reduction. The wound is sutured, and stitches are removed after one week.

 

During the postoperative period, brushing of the surgical site should be avoided. Plaque control is achieved through short-term use of antiseptic solutions or gels, without the need for antibiotic therapy.

 

If a prosthetic restoration (crown, veneer, or bridge) is planned after the procedure, it is necessary to wait 3 months for the tissue to fully heal.
