What is a granuloma, what are the symptoms, and how is it treated?

 

A granuloma is an inflammatory process that develops at the tip of the tooth root, most often as a result of untreated or inadequately treated tooth decay. Bacteria penetrate the root canals of the tooth and affect its internal structures – first the dentin, then the pulp – which ultimately leads to nerve inflammation.

 

In the early stages, a granuloma may be asymptomatic, so patients are often unaware of its presence. If the condition worsens, it becomes acute, and the following symptoms may occur.

What is endodontic treatment?

 

Endodontic (root canal) treatment – also known as “nerve treatment” – is a dental procedure used to save a tooth affected by deep inflammation or infection of the dental pulp. This procedure prevents the spread of infection and further complications, preserves the natural tooth, and ensures a long-term healthy smile.

 

Endodontics is performed when the dental pulp is irreversibly damaged due to bacterial penetration into the tooth as a result of various pathological processes, such as:

 

  • Deep tooth decay

  • Tooth trauma

  • Tooth fracture or crack

  • Recurrent infections beneath an old filling

 

In many cases, the patient may not immediately realize that there is a nerve problem, since symptoms can be mild or unclear. However, if left untreated, the inflammation can lead to complications such as abscesses, granulomas, and even tooth loss.

If a tooth is properly treated and restored, it can last for many years – even a lifetime!

How does endodontic treatment work?

 

During the initial consultation, the dentist reviews your X-ray to determine the extent of the damage and plan the therapy. The decayed portion of the tooth and the infected pulp are removed under local anesthesia. The root canals are then thoroughly cleaned, disinfected, and shaped to eliminate bacteria and infected dentin.

Endodontic retreatment

 

The success of endodontic therapy is monitored over time. Immediately after the procedure, a control X-ray is taken to confirm that the canals have been properly filled to the tip.

 

At the follow-up appointment, usually 6 months later, another X-ray is performed to check whether the periapical lesion around the treated tooth root is healing.

 

Endodontic treatment has a very high success rateover 90%. However, in rare cases, infection may return – most often due to complex canal anatomy or leakage of the filling. In such cases, retreatment (repeating the procedure) may be indicated.
