Why and how should a tooth be restored after root canal treatment?

 

After treatment, the tooth is left without a nerve, with reduced sensation, and often with significantly weakened structure.

 

Depending on the degree of damage and functional load, the dentist will choose the optimal method of restoration – ranging from direct composite fillings, to adhesive ceramic restorations, to ceramic crowns with or without a core build-up.

When most of the crown of the tooth is preserved, it is possible to restore it with a direct composite filling. This type of restoration is carried out immediately in the dental office, in a single visit, without the need for a laboratory phase.

 

Direct restorations are particularly suitable for front teeth, as well as for small to moderate defects in back teeth where no major cusps or supporting walls have been lost. Properly placed composite can successfully restore the tooth’s shape and function while preserving as much healthy tissue as possible.

 

However, when teeth are significantly weakened, the risk of fracture increases – especially in molars exposed to strong chewing forces – and in such cases, indirect ceramic restorations are recommended.

 

Indirect Restorations

Indirect restorations are prosthetic replacements that are not fabricated directly in the patient’s mouth but are made in a dental laboratory based on a precise digital impression of the tooth. Once fabricated, the restoration is permanently cemented onto the prepared tooth. Thanks to the fabrication method and premium materials, indirect restorations provide high precision, strength, and aesthetics.

 

For more extensive damage, especially in posterior teeth, an excellent solution is a lithium disilicate ceramic overlay. An overlay covers almost the entire chewing surface of the tooth, protecting it from fracture while preserving more healthy tooth tissue compared to a full crown. Thanks to its adhesive bond with the remaining tooth, an overlay strengthens the structure and ensures long-term stability, function, and aesthetics.

 

In cases where the tooth is significantly weakened but the root structure is stable, an endocrown is recommended. This is also an indirect restoration made of lithium disilicate ceramic, anchored inside the root canal portion of the tooth and covering the entire crown portion. It combines the strength of a full ceramic crown with the benefits of adhesive techniques, offering reliable protection without the need for an additional build-up.

 

When tooth structure loss is extensive and the remaining structure cannot safely support a restoration, a post-and-core is used. A fiber-reinforced post (most often made of glass fibers) is cemented into the root canal of the treated tooth to provide retention for the core. A crown is then placed on top, ensuring functional and mechanical stability.

What determines the choice?

 

The choice of post-endodontic restoration is based on individual assessment. Direct composite fillings are suitable for simpler cases, while lithium disilicate overlays and endocrowns offer the best balance of strength and tissue preservation. When that is no longer feasible, a post-and-core with a crown remains a reliable solution.
